A finding with an owner
A dashboard is useful when someone can interpret a change and act on it. For cancellation cases, the service owner needs to know whether an increased delay comes from waiting for learner agreement, partner confirmation or a handover that nobody has accepted. The four process areas overlap: an operating result can prompt a new requirement or a change in how quality is judged.

Choose how often to collect and review observations according to the event and the cost of detecting a problem late. A small trial may answer the question; retain enough observations to support the conclusions drawn from it. Bring owners together when a measure covers several teams. Automate collection with checks on missing or duplicated events. Decide who can change a target and how findings inform wider design decisions.
