Replace prompts with your own decisions, sources and results. Link existing records where useful.

## Scope and current work

Identify the service, the proposed release and the unfinished work it affects.

## Readiness and acceptance

Name the owners and required checks. Keep expected and actual results
separate, and state who may accept the change into service.

## Support and response

Name support contacts and cover. State who can act during an incident and
who communicates with affected people.

## Recovery and continuity

Describe how records and service operation would be restored. State how
records would be reconciled with work already done and who may accept
resumption.

## Improvement and retirement

Record the causes found during review and the changes agreed. Name the
owners of remaining work and obligations when the service is retired.
